for normal bar to fat bar adaptor, these risers are very common, less than $30 available from LAB to Chong Aik however the finishing is not very good.

 

if you need a more than 1" riser, a good brand is the Rox Riser. Very high quality but come with price.

Look for :

 

"........7/8" stem clamp x 7/8" handlebar"

 

and they also have those that can clamp both regular bars and fatbars with the use of reducers

i believe max ht universal riser is around 3cm / 30mm ..

some custom ones goes up to 35mm (think M-tech have these)
