Ride up to Kuala Trengganu (KT), about 600km from Singapore, stay 1 nite. (8 hours plus stops). The next day, head north towards Kota Bahru, and look for Macang which is on your left. It should take 1.5 hrs after you leave KT. Head towards Jeli. This is where the fun begins.

Jeli to Gerik, is what was referred to as Gerik Highway, or the correct term "East West highway". Its nothing like the NS Highway. This is one lane highway that snakes around mountains and if not for the humid climate, you might be thinking you're riding in Europe. You can choose to stop at Pulau Banding, an island situated in the middle of the highway.

From here, you could proceed to Penang via Baling or head south and stay the nite at Ipoh or Cameron Highlands via Kuala Kangsar.

What KISS recommended is a ride from the East Coast (which is awesome too, you can enjoy the seaside ride on Kuantan), but I took the NSH highway.

From Singapore, head north to Ipoh. I suppose you don't need directions from Singapore to Ipoh/KL stretch. I'd recommend you to rest 1 night at Ipoh, eat the nice dim sum there and buy some pomelo from the chio bu. Set off really early because Route 4 aka East West Highway aka Jeli-Gerik Highway doesn't have phone reception and it is very foggy and damp at night.

Back on NSH, go straight from Ipoh all the way north until you hit Kuala Kangsar exit. From Ipoh to Kuala Kangsar is about 30km.

Go straight all the way after you exit Kuala Kangsar, you will pass through some village, country roads and all the way to Gerik town after 80km. Pump up your petrol here because I don't remember there is petrol station on Route 4. Recommend to eat seafood at Hock Soon (i think) Seafood Restoran, awesome. Make your fill and proceed.

Moving off from Gerik, not long after, about few hundred metres, you will come to a junction which you will turn right. Towards Jeli, Route 4.

Continue about 80km of uphill & twisties, keep to Route 4 towards Jeli if you see any split road. You should be coming to the top of the Kelantan mountain, there is a rest station here where you can take some photos, go to toilet and buy some snacks. You should be at the fringe of Kelantan state. Beware of elephants crossing while travelling on Route 4.

After the 100km on route 4, it should be downhill, becareful for local racers. Coming downhill, you should see petrol station on your right. People at Kelantan do not speak english at all, it's a muslim state. If you are a malay then no problem.

You'll come to a junction at about 110~120 km from Gerik. If you want to have more corners through the villages (dangerous though, kids and motorcycles run out like nobody's business), take D22 on left, else just go straight.

You have finished the mother of all riding roads (at least that's what AAS magazine says..). Straightahead for about another 10~20km is still route 4, but nothing spectacular.

Probably another thing to take note @ Gerik. During rain at Gerik, very poor visibility and lots of water. i.e. heavy downpour. If you want to ride there, do take note of the recent weather and don't get caught in the rain.

If you plan your ride from Malaysia East Coast, one area you may consider entering Betong, Thailand. Can nuar there or explore some places of interest.

Ah yes... SV650 made a very good point. During my stay in Kota Bharu for 5 days, the rain is no joke. Singapore rain at most 1 full day, then stop at the night, maybe next day rain again.

In Kelantan, NO WAY! It can rain consecutively without stopping for a second, simultaneous heavy rain for few days. Until all the roads flood.
